Short answer: A clinic can send WhatsApp appointment reminders by using Meta’s official WhatsApp Business Platform, getting the patient’s opt-in when they book, and sending a pre-approved reminder template before the visit. AiBukko, the WhatsApp platform by Vraja Labs, sends these reminders automatically from the same workspace where the booking was made.
Most missed clinic appointments are not deliberate. A patient booked a follow-up two weeks ago, life got busy, and the day slipped past. The doctor’s slot stays empty, the next patient who wanted that slot was turned away, and the front desk spends the next morning calling people who did not turn up. A short, well-timed reminder on the app patients already open every day is one of the simplest fixes a clinic can make.
This guide covers what a clinic in India needs to know: how WhatsApp reminders work on the official API, the rules Meta sets, what to say, when to send it, and how to handle patient data carefully.
Why clinics are moving reminders to WhatsApp
Clinics have traditionally reminded patients by phone call or SMS. Both work, but both have costs. Calls take staff time, often at the busiest hour of the morning. SMS reminders arrive from a sender header that many patients don’t recognise, among dozens of promotional texts.
WhatsApp changes three things:
- Recognition. The message comes from your clinic’s verified business profile and name, not an unknown number.
- Two-way conversation. If the patient has a question (“Should I come fasting?”), they can reply in the same chat and your front desk can answer.
- Less manual work. Once reminders are automated, staff stop ringing through the day’s list by hand.
We are not going to quote a no-show reduction figure here, because results depend on your clinic, your patients and your reminder timing. The honest way to know is to measure your own no-show rate for a month before and after you start.
How WhatsApp reminders work on the official API
There are two WhatsApp products for businesses: the WhatsApp Business app (a phone app for one-to-one chats) and the WhatsApp Business Platform, which developers often call the WhatsApp API. Automated reminders at clinic scale run on the Platform. (We compare the two in WhatsApp Business API vs the WhatsApp Business app.)
On the Platform, two rules shape every reminder:
- Business-initiated messages must use an approved template. Meta’s WhatsApp Business Messaging Policy says businesses “may only initiate conversations using an approved Message Template.” A reminder is usually sent days after the patient last messaged you, so it is a template message.
- The 24-hour customer service window. When a patient messages your clinic, a 24-hour window opens, and it resets with each new patient message. Inside that window your team can reply freely. Outside it, only approved templates can be sent.
Templates are reviewed by Meta. Meta’s documentation says review can take up to 24 hours, and a template’s status can change later based on how people respond to it. Appointment reminders typically fall into Meta’s utility category (messages about a transaction or service the person has asked for), rather than marketing. Meta assigns the final category according to its template guidelines.
Where AiBukko fits
AiBukko connects to Meta’s official WhatsApp API and handles the parts a clinic doesn’t want to build:
- Bookings in chat. Patients can book through the AiBukko WhatsApp bot, or your team can add a booking. See WhatsApp booking.
- Automatic reminders. AiBukko sends the reminder template before each appointment. See WhatsApp appointment reminders.
- Rescheduling. The bot handles rescheduling requests so a patient who can’t make it moves their slot instead of simply not turning up.
- Feedback after the visit. AiBukko can collect feedback once the appointment is over.
- One shared inbox. When a patient replies with a question, any member of the front desk can answer from the shared multi-agent inbox.
Plan your reminder timing and wording with your front desk before you switch on, so patients get a message that matches how your clinic actually runs.
Step by step: setting up reminders for your clinic
- Get opt-in when the patient books. Meta requires that people have given you their number and agreed to receive messages from your business. The opt-in must clearly name your clinic. A line on your booking form or registration sheet (“I agree to receive appointment updates from [Clinic name] on WhatsApp”) covers this. Meta lists website, SMS, phone (IVR) and in-person or paper opt-in as supported methods.
- Write your reminder template. Keep it short and factual (examples below), then submit it for approval. Templates are reviewed by Meta before they can be sent.
- Decide timing. Many clinics send one reminder the day before and, for early slots, another on the morning of the visit. Test what works for your patients.
- Give patients an easy way to change plans. A reminder that makes rescheduling easy recovers slots that would otherwise go empty.
- Train the front desk on the inbox. Replies to reminders come into the shared inbox, so agree who answers what.
- Measure. Track no-shows per week before and after.
Reminder templates you can adapt
Use placeholders for the patient’s name, doctor, date and time. Keep medical details out of the message (see the privacy section below).
Day-before reminder
Hi {{name}}, this is a reminder of your appointment with {{doctor}} at {{clinic}} tomorrow, {{date}}, at {{time}}. Reply here if you need to reschedule.Same-day reminder
Good morning {{name}}. See you today at {{time}} at {{clinic}}. Please arrive 10 minutes early for check-in.Follow-up visit reminder
Hi {{name}}, {{doctor}} asked us to remind you that your follow-up visit is due. Reply here and we’ll find a time that suits you.Feedback after the visit
Thank you for visiting {{clinic}} today, {{name}}. How was your experience? Reply with a rating from 1 to 5.
Meta reviews every template, so wording may need small changes before approval.
Timing and tone: what works in practice
- Don’t send too many. One or two reminders per appointment is plenty. Patients can block or report a business, and Meta’s systems can limit businesses whose message quality stays low.
- Respect the hour. Avoid sending very early in the morning or late at night.
- Use the patient’s language. If most of your patients prefer Hindi or a regional language, write the template in that language. Meta lets you create templates in many languages.
- Say who it’s from. Your clinic’s name should be in the message, even though it also shows on the profile.
Patient privacy: WhatsApp policy and the DPDP Act
Clinics handle sensitive information, so be careful about what goes into a reminder.
Meta’s policy. The WhatsApp Business Messaging Policy says businesses must not use WhatsApp to send or request health-related information where applicable regulations prohibit sharing it with systems that don’t meet heightened requirements for health data. It also says not to ask people to share full ID card numbers or other sensitive identifiers. In practice: a reminder should say when the appointment is, not what it is for. “Your appointment with Dr Rao” is fine; diagnosis, test results or treatment details are not.
India’s Digital Personal Data Protection Act, 2023. A patient’s name and phone number are personal data under the DPDP Act. The Act requires a clear notice when you collect personal data and, for consent-based processing, consent that is freely given, specific, informed and unambiguous, and that can be withdrawn as easily as it was given. MeitY notified the DPDP Rules, 2025 in November 2025, with most obligations coming into force in phases after that. Speak to your legal adviser about what applies to your clinic and when.

Common mistakes to avoid
- Messaging patients who never agreed to WhatsApp messages.
- Using unofficial “bulk sender” tools instead of the official API. These break Meta’s terms and can get your number restricted.
- Putting test results or diagnoses in a reminder.
- Sending reminders with no way to reply or reschedule.
- Forgetting to update the reminder when a patient reschedules.
Frequently asked questions
Do patients need to opt in before a clinic sends WhatsApp reminders?
Yes. Meta’s policy says you can contact people only if they have given you their number and agreed to receive messages from your business. Collect this when they book.
Can a clinic send reminders from the WhatsApp Business app?
You can message patients one by one from the app, but automated reminders for every appointment are built on the WhatsApp Business Platform (the API), which is what AiBukko uses.
What should a clinic reminder not include?
Diagnoses, test results, treatment details and ID numbers. Keep it to the patient’s name, doctor, date, time and clinic.
Can patients reschedule from the reminder?
AiBukko’s bot handles rescheduling in the chat, so a patient can reply to the reminder and move the appointment.
Can AiBukko collect feedback after the visit?
Yes. AiBukko can send a short feedback request once the appointment is over.
